Cards Plus, Inc - Tribute Cards

Whether it’s a Birthday or a Sympathy card: every dollar raised from these cards goes directly to the UM/Sylvester Comprehensive Cancer Center for cancer research.

Each card is personalized according to your instructions on the order form. There is a $15 minimum donation per card, and a group of very special cards for a donation of $25 or more.

Instructions:

• Write down card number of each of the card(s) you want to order.
• Email your order request to LMiller126@aol.com.
• Include the card number(s).
• State the quantity of each card number(s) you are ordering.
• The name(s) of the person(s) you are honoring.
• The name(s) and mailing address(es) of the person(s) to mail the card.I
• f the card you selected is not currently available, we will automatically send out a similar card.
